Disciplinary Records Request

The Dean of Students Office at Missouri S&T is committed to empowering every student and supporting their holistic success. We understand that disciplinary records requests are often part of important transitions in the academic and professional journey. Our goal is to provide a clear, transparent process that supports continued success and development.

 

The Dean of Students Office maintains and provides access to disciplinary records for current and former students. We are the centralized reporting repository for:

  • Academic conduct records
  • Equity and Title IX matters
  • Individual conduct incidents
  • Student organization conduct

Our standard Conduct History Report includes:

  • Case number
  • Incident date
  • Violations with findings of responsibility  
  • Sanctions issued and completion status
  • Eligibility to (re)enroll

If no records exist, reports state: "NO RECORD TO REPORT – INDIVIDUAL ELIGIBLE TO REENROLL BASED ON CONDUCT HISTORY."

If a case is pending procedural process, reports state: “**** CASE PENDING PROCEDURAL PROCESS****.”

Students may initiate the process by:

  • Completing our online form
  • Submitting a signed disclosure for the individual
  • Allowing 2-4 business days for processing

The length of time we maintain, and report records depends on the outcome of the case.

The Dean of Students Office cannot provide:

  • Enrollment verification
  • Academic records or grades
  • Degree confirmation
  • Financial information
  • Scholarship details
